Plan deductible waived for three office visits for illness or injury each plan year and an additional three visits for alternative care pre-deductible.

Beneficial Rx Plan - (PPO)
Plan deductible waived for three office visits for illness or injury each plan year, three visits for alternative care pre-deductible, and prescriptions covered at $15 and 50%.

Plus Plan - (POS)
Low out-of-pocket expenses when receiving primary care services.

Preferred Plan - (PPO)
Freedom in provider selection at a reasonable monthly rate.

Traditional Plan
Complete flexibility in provider choice.

Health Savings Account (HSA) Plan
High deductible health plan. When paired with a Health Savings Account you can save for qualified health expenses on a tax-free basis.
