The Plus Plan is an individual health benefit plan that waives your deductible for primary care provider services such as office visits. This plan will pay a higher level of benefit when you choose a participating provider within our extensive network. However, you may receive care from any eligible provider, regardless if they are participating or not. In the event you see a non-participating provider, your out-of-pocket expenses will be higher.

Plan Highlights

First-dollar preventive care; illness, accident and prescription drug coverage
Deductible waived for certain services provided by the Primary Care Physician (PCP)
$20 co-pay for chiropractic, acupuncture and naturopathic care
Annual deductible of $1,000
Prescriptions covered at $15 or 50%, whichever is greater; not subject to the deductible

This plan is designed to meet the needs of individuals and families who want to lower their out-of-pocket expense when receiving PCP services. The Plus Plan pays 80% of all eligible services with no deductible for PCP services. This plan pays 100% of expenses over and above the plan year out-of-pocket maximum.

The Plus Plan is an individual health benefit plan available for Oregon residents under age 65 who are not eligible for Medicare. Eligible individuals include you, your legal spouse and any unmarried children under age 19 or 23, depending on certain conditions.
