The Preferred Plan offers you greater freedom in provider selection. The Plan pays a higher level of benefit when you choose a participating provider within our extensive network, which offers over 12,000 participating providers to choose from. However, you may receive care from any eligible provider, regardless if they are participating or not. In the event you see a non-participating provider, your out-of-pocket expenses will be higher.

Plan Highlights

First-dollar coverage including women’s preventive care visit, and deductible waived for treatment received within 90 days of an accident
Chiropractic, acupuncture and naturopathic care covered after the deductible, payable up to $1,000 per person per year
Annual deductible choices of $1,000, $2,500 and $5,000
80% reimbursement of prescriptions after the deductible is met

This plan is designed to meet the needs of individuals and families who do not have other healthcare coverage and are looking to purchase benefits on their own. The Preferred Plan pays 80% of all eligible services after the deductible and 100% of expenses over and above the plan year out-of-pocket maximum.

The Preferred Plan is an individual health benefit plan available for Oregon residents under age 65 who are not eligible for Medicare. Eligible individuals include you, your legal spouse and any unmarried children under age 19 or 23, depending on certain conditions.
