ODS Individual Dental Exchange

Did you know that individuals and families who have previously been enrolled on an ODS group dental plan are eligible for the ODS Dental Exchange plan? This plan allows members to "exchange" their group dental policy for an individual dental policy. Prior ODS group members are offered this product automatically at the time they lose coverage. If the member elects COBRA, they will again be offered the dental exchange at the time they come off of COBRA. The Dental Exchange benefit summary, application, and return envelope are automatically sent to the member's home upon termination off a group policy.

Underwriting Requirements for Individual Dental Exchange:

Must be an Oregon resident and live in Oregon at least six months out of the year
Must have been previously enrolled in an ODS employer-sponsored dental plan for at least 12 continuous months
Eligible members have 90 days from the date the employer-sponsored coverage ended to enroll
