North Bend Willamette Dental office closing July 31

We wanted to let you know of an upcoming change in the Willamette Dental location options for members. As of July 31, 2009, Willamette Dental will be closing the North Bend location. To ensure members experience no interruption in care, they can choose from the following options:

If members wish to continue with Willamette Dental, the closest offices to the North Bend location are in Eugene and Roseburg.

If members choose to move to another ODS plan with dental offices located in the North Bend area, they can do so during the special enrollment period, which will be 6/15 through 7/15 for an 8/1/2009 effective date.

If the member selects another ODS plan, the member should request their dental files from the North Bend Willamette Dental office and bring their materials with them to their new dentist.

We will be communicating with groups and members affected by the North Bend office closure to let them know about their options.
