Voluntary Dental - Eligibility and Underwriting
Requirements
Plan Requirements
- Minimum group size of 51 eligible employees or at least 26 enrolled
subscribers.
- Employers to set up payroll deductions and submit premium on behalf
of their employees.
- ODS Voluntary dental plans cannot coexist with another dental plan
offered by another carrier.

Eligibility
- The plan requires a waiting period of the 1st of the month following
90 days of employment, unless ODS is replacing prior dental coverage.
- Eligible employees and their dependents may initially enroll during
annual open enrollment.
- Dependents must be enrolled at the time the eligible employee initially
enrolls, or at the time of a qualifying event. The eligible employee
must be enrolled in the plan to add dependents to the coverage.
- If coverage is dropped at any time the employee and/or dependents
may not re-enroll in the plan.

Participation Requirements
ODS requires a participation level of at least 35 percent for groups
with 51 or more employees or at least 26 enrolled subscribers.
Participation requirements will be credited five percent if the group
complies with the following;
- Executive from employer group endorses the ODS Voluntary dental
program.
- A memo/ brochure of the ODS Voluntary dental plan is distributed
to all eligible employees.
- ODS Marketing Service Representative works with employer group
to schedule and conduct mandatory enrollment meetings.
- All eligible employees complete an ODS enrollment/declination form.

Contribution Requirements
Employee contributions must be up to 100% of the premium.
