I am losing my ODS employer-sponsored dental plan. Is there any other dental plan available for me?

Yes. The ODS Individual Dental Exchange plan is being offered to anyone who was previously enrolled in ODS employer-sponsored dental plan and had at least 12 (continuous) months coverage. This plan allows you to "exchange" your dental policy for an individual dental policy.

How does ODS determine the effective date of my ODS Individual Dental Exchange policy?

Once a completed application with future payment method selection and premium are received, your policy is effective the first of the month following receipt of your application.

When will my ID card(s), policy, and member handbook(s) be received?

You will receive your individual plan ID card(s), member handbook(s) and Declaration page, which summarizes the enrollment, within 30 days from underwriting acceptance. If you have not received these items within 30 days from underwriting acceptance, please contact ODS at (503) 243-3944 in the Portland area or (877) 277-7073, toll free between 7:30 a.m. and 5:30 p.m.

Can I terminate my Individual Dental Exchange policy at any time?

Yes. You may terminate your dental exchange policy upon providing ODS with a 30-day advance written notice. Insurance will end on the last day of the monthly period through which premiums are paid.
