Can I purchase just the Individual Options Dental Plan?

The ODS Individual Options Dental Plan is only available with the purchase of an ODS Individual medical plan. The only time to select the dental coverage is upon initial enrollment in an ODS individual medical plan. There is no opportunity to add the dental later or reinstate it should it be cancelled.

How does ODS determine the effective date of my individual medical policy?

We must receive your application by the 20th of the month for consideration for the next month's effective date beginning on the 1st of the month following underwriting approval. Applications received after the 20th will be processed for the subsequent month's effective date on the 1st. If a 15th effective date is requested, we must have your application by the 5th day of the requested month. Applications received after the cutoff date will be processed for the subsequent month's effective date.

Once all underwriting requirements including complete application with future payment method selection, premium, and health history information are received (all dates of treatment, medications, and physicians during the past 5 years), your policy is effective the first of the month following ODS' Underwriting approval notice.

If a 15th effective date is resulting, we will bill for an additional two weeks premium to bring you to a first of the month cycle thereafter.

When will the individual plan ID card(s), welcome letter, and member handbook(s) be received?

You will receive your individual plan ID card(s), policy, member handbook(s) and declaration page, which summarizes the enrollment, within 30 days from underwriting acceptance. If you have not received these items, please contact ODS at (877) 277-7073, toll free between 7:30 a.m. and 5:30 p.m.

If I am a current individual member, can I change from one individual plan to another?

Yes. You may switch plans with ODS as follows:

Can I terminate my individual medical policy at any time?

Yes. You may terminate your individual medical policy or the medical coverage for an insured dependent upon providing ODS with a 30-day advance written notice. Insurance will end on the last day of the monthly period through which premiums are paid.

Individual Options Dental plan members and dependents may not retain their dental coverage if their coverage in an ODS individual hearth benefit plan ends.

If the treatment is for an accidental injury, include a statement explaining the date, time, place, and circumstances of the accident when you send us the physician or professional provider's bill.
