What if I need care for a condition and there is no specialist in the network?
The Community Care Network is a comprehensive network of primary care and specialty providers. In the event that there is no CCN provider for a specialty, our team will facilitate the process of locating a provider and extend in-network benefits. The ODS Care Coordinators are available to help identify providers and assist in directing your care.
Does my whole family have to enroll in the Community Care Plan (CCP)?
A member's family will be required to enroll in the CCP. However out-of-area dependents have the option to enroll in an alternate network. For more information, please contact ODS Customer Service.

The CCN encompasses a comprehensive network of providers and facilities surrounding Legacy Health and Salem Health. Providers and facilities in the CCN provide high-quality, individualized care with proven results.
With Plan 4, you'll get a more comprehensive healthcare plan that enables you to engage with providers and ODS Care Coordinators. Each is focused on helping you address your specific health needs.
What happens if a provider refers me to a provider not in the Community Care Network?
We are working with providers to ensure they refer members to CCN providers. Services received by a non-CCN-participating provider will be paid at the out-of-network benefit level.

What does "more integrated care" mean?
As a member of the Community Care Plan, you will benefit from a coordinated team approach, including a high level of personal service, more engagement with providers and ODS Care Coordinators, and an overall better experience on your health journey.
How can I get care while traveling? Will my benefits be paid in- or out-of-network?
You will have access to the ODS Travel Network. The ODS Travel Network allows medical plan members to receive emergency and non-emergency care outside of their primary service area while traveling. As an eligible member seeking care through a PHCS Healthy Directions provider, you will receive in-network benefits. If you have dependents living outside the primary network area, your dependents can also use the ODS Travel Network to receive care at an in-network benefit level.
Please note, the ODS Travel Network is not an alternative primary network. Members must seek in-network services whenever possible, and preauthorization is required for in-patient services.
Is a referral required to see a specialist?
There are no referrals currently required on the CCN plan.

Does all my care have to be within the Community Care Network?
The CCN provides a comprehensive network of providers, aimed to support the spectrum of healthcare needs. If a specialty service cannot be supported by the CCN, ODS Care Coordinators will help you identify providers and assist in directing your care. We will then extend in-network benefits for these providers.
Can I change plans during the year if I don't like the Community Care Network or if my provider drops out of the network?
Members selecting the Community Care Network will have the opportunity to change plans at the next open enrollment period.
If you move out of the service area, your school district or educational entity will assist in providing plan options in your new area.
