There may come a time when you want to leave, or are required to leave, the PERS ODS Advantage PPORX plan. “Disenrollment” means ending your membership in the PERS ODS Advantage PPORX plan. Disenrollment may be voluntary (your choice) or involuntary (not your own choice):
Until your membership officially ends, you should keep receiving your Medicare services through PERS ODS Advantage PPORX or you will have to pay more for your services.
PERS ODS Advantage PPORX is sponsored by the PERS Health Insurance Program. Disenrolling from PERS ODS Advantage PPORX will disenroll you from PERS. Please call the PERS Health Insurance Program at 503-224-7377 or toll free at 1-800-768-7377 (TTY users call 711) from 7:30 am to 5:30 pm Monday through Friday to discuss your options. If you leave PERS Health Insurance Program you may not be able to return to the PERS Health Insurance Program.
