There may come a time when you want to leave, or are required to leave, the PERS ODS Advantage Rx plan. “Disenrollment” means ending your membership in the PERS ODS Advantage Rx plan. Disenrollment may be voluntary (your choice) or involuntary (not your own choice):
Until your membership officially ends, you should keep receiving your Medicare prescription drugs through PERS ODS Advantage Rx.
PERS ODS Advantage Rx is sponsored by the PERS Health Insurance Program. Disenrolling from PERS ODS Advantage Rx will disenroll you from PERS. Please call the PERS Health Insurance Program at 503-224-7377 or toll free at 1-800-768-7377 (TTY users call 711) from 7:30 am to 5:30 pm Monday through Friday to discuss your options. If you leave PERS Health Insurance Program you may not be able to return to the PERS Health Insurance Program.
Whether leaving the plan is your choice or not, please review the Ending your membership in the plan section of our Annual Notice of Changes - Evidence of Coverage for the PERS ODS Advantage Rx plan. This section explains your Medicare prescription drug coverage choices if you want to leave the plan and the rules that apply. You can call customer service if you need help.
