ODS Advantage PPO enrollment information

To be eligible for this plan, you must meet the following requirements:

You must have both Medicare Part A and Part B
You must reside in the state of Oregon
You must continue to pay your Part B premium (if not paid under Medicaid or third party)
You cannot enroll if you have End Stage Renal Disease (ESRD). If you are currently enrolled in an ODS medical plan, you may be able to enroll even if you are having renal dialysis or have had a recent kidney transplant. Contact member services for more information.

You can enroll in ODS Advantage PPO, when you become eligible for Medicare, when you have a Special Election Period and each Annual Election Period (October 15th through December 7th each year). For more information about when you can enroll or if you have questions about enrolling, please call member services.

Your coverage will generally be effective the first of the month following our receipt of a "complete" enrollment form. If you submit an incomplete enrollment form, we will contact you for more information. This can delay your effective date.

Warning: If you are enrolled in another Medicare Advantage plan, or a standalone Part D plan, enrolling into ODS Advantage PPO will disenroll you from that plan and you may not be able to reapply until the next Medicare Annual Election Period October 15 through December 7 of each year.

Note: If you are already enrolled in another Medicare Advantage Prescription Drug plan, you must receive your Medicare prescription drug benefit through that plan.

Ways to Enroll

Medicare beneficiaries may enroll in ODS Advantage PPO through the CMS Medicare Online Enrollment Center. You will be forwarded to the official U.S. Government site for Medicare, http://www.medicare.gov/.
Fill out the ODS Advantage PPO online enrollment form.
Print the ODS Advantage PPO enrollment Form and Instructions and send it to the address on the form.
Call member services to request an enrollment packet.
