ODS Advantage PPORX Select (PPO) disenrollment
There may come a time when you want to leave, or are required to leave, the ODS Advantage PPORX Select plan. “Disenrollment” means ending your membership in the ODS Advantage PPORX Select plan. Disenrollment may be voluntary (your choice) or involuntary (not your own choice):
- You might leave the ODS Advantage PPORX Select plan because you have decided that you want to leave.
- There are a few situations in which we are required to end your membership. (For example, if you permanently move out of our geographic service area.)
Until your membership officially ends, you should keep receiving your Medicare services through ODS Advantage PPORX Select or you will have to pay more for your services.
Whether leaving the plan is your choice or not, please review the Ending your membership in the plan section of our Combined Annual Notice of Changes — Evidence of Coverage for the ODS Advantage PPORX Select Plan. This section explains your Medicare coverage choices if you want to leave the plan and the rules that apply. You can call member services if you need help or to ask for a disenrollment form
