Eligible members must be an Oregon resident and live in Oregon at least six months out of the year. If a member moves outside of Oregon while on this plan, they will automatically be terminated from the plan the first of the month following the move.
Applicants must be approved by ODS Underwriting using the Oregon Standardized Health questionnaire. Effective dates are assigned by Underwriting and are generally the first day of the month following Underwriting approval. Producers and applicants are required to submit a clean and complete application a minimum of 10 days prior to the requested effective date. Applications received after the cut-off date will be processed for the subsequent month’s effective date.

Eligible members must reside in the state of Oregon and be enrolled in an individual medical plan. Members must choose the dental plan upon original enrollment in one of the individual medical plans (see “Underwriting” for additional dental plan eligibility rules).

Dental plan members must choose the dental plan upon originally enrolling. Members will not be able to select the dental plan at a later date. If a member drops the dental plan rider, it cannot be reinstated. If a member drops an individual medical plan, he or she cannot retain the dental plan rider, however they can apply for an individual dental plan.
