The Community Care Network encompasses a comprehensive network of providers and facilities surrounding Legacy Health and Salem Health.

Salem Health

Providing quality and compassionate healthcare, Salem Health includes Salem Hospital, West Valley Hospital and a group of medical practices called Willamette Health Partners with clinics throughout the region. Our mission is to improve the health and well-being of the people and communities we serve.

 

Salem Hospital

Licensed for 454 beds, Salem Hospital is the fourth largest hospital in Oregon, according to the most recent Book of Lists by the Portland Business Journal.

More than 465 providers are on the active medical staff representing 46 different specialties. The hospital's emergency department, a Level II Trauma Center, is the busiest in Oregon.

Salem Hospital is among six percent of hospitals nationwide that can claim Magnet® recognition for exemplary patient care. Numerous other awards recognize the high-quality care we offer and successful health outcomes for patients.

Bariatric Surgery Center

The Bariatric Surgery Center provides patients with options for surgical weight loss including gastric bypass and the laparoscopic gastric band system. The Salem Hospital Bariatric Surgery Center has been designated as a Bariatric Surgery Center of Excellence® with its demonstrated track record of favorable outcomes in bariatric surgery.

Heart & Vascular Center

From diagnostic testing, emergent treatments and cardiac rehabilitation, you can look to the Heart & Vascular Institute to help you return to good health. Within the center, 21 heart specialists team together with Salem Hospital to form the Heart & Vascular Institute, offering comprehensive treatment of irregular heart rhythms, blockages in the heart, peripheral vascular disease and more. Salem Hospital's open-heart surgery and valve-repair programs are among the most successful in Oregon.

Orthopedic services

As the state's number one provider of hip and knee replacements, Salem Hospital treats and rehabilitates patients with complex to minor orthopedic conditions. Our "Active You" Joint Replacement Center of Excellence helps patients with pre-surgery education through surgery and post-surgery rehabilitation. Our orthopedic rehabilitation home specialties include hand, physical, occupational, balance and aquatic therapy.

Salem Cancer Institute

The Salem Cancer Institute is a nationally accredited breast center and provides compassionate care for all types of cancer. Our specialists, state-of-the-art technology and cancer care coordinators all support the patient throughout the cancer journey.

Salem Health ranks in the top 3 percent nationally for the use of electronic medical records. Patients of Salem Hospital, West Valley Hospital and Willamette Health Partners can easily manage their healthcare online with MyChart @ Salem Health.

West Valley Hospital

West Valley Hospital provides compassionate care with its 24-hour emergency department, cardiovascular care, imaging, lab, surgical and rehabilitation services. Affiliated clinics Monmouth Medical Center and Physicians & Surgeons, provide primary care and the Surgical Specialty Clinic provides general surgery, gynecological and urological care.

Willamette Health Partners

Willamette Health Partners provides primary and specialty care in Salem, Keizer, Monmouth and Dallas. Our providers include family medicine physicians, ob/gyns, nurse-midwives, general surgeons, perinatologists and pain management specialists.

If you need a primary care provider, our physicians at Willamette Health Partners Family Medicine can often accommodate same-day appointments.

Willamette Health Partners specialists practice all facets of obstetrics and gynecology, maternal-fetal medicine, midwifery, pain management and general surgery.

Our patient-centered medical home model encourages an interactive provider-patient relationship, giving patients the information and tools needed to make healthy lifestyle choices.

A medical home focuses on the health of the whole person, rather than treating separate symptoms and illnesses. All our clinics have adopted or are transitioning to this model, which we will continue to refine over the coming months

About medical homes - A medical home offers services tailored to your needs. These range from reminders about prevention and wellness, to care focused on helping you manage chronic conditions such as diabetes or high blood pressure. Patients with complex needs receive extra support.

Maternity and childbirth: For those starting families, Legacy has family birth centers and newborn nurseries at all five medical centers, plus specialty services for patients who want or need them, including:

Children's services: Children have different healthcare needs than adults, which is why Legacy Health offers specialized care in an environment designed for young patients. More than 600 doctors, including pediatric medical and surgical specialists and community physicians affiliated with the Children’s Hospital at Legacy Emanuel and its extension at Legacy Salmon Creek in Vancouver provide the region’s most advanced care, including:

A new, state-of-the-art building, opening February 2012, will include 165 private rooms with a pull-out sofa bed so a relative can stay the night, a 25-bed day surgery unit, a neonatal intensive care unit connected with a sky bridge to Legacy Emanuel’s labor and delivery unit, and a new children’s emergency room double the size of the current space. Award-winning care: Many of Legacy's specialty services have won national quality awards, and meet or exceed the highest national benchmarks.

Women's Heatlh: In addition to maternity services, Legacy offers GYN specialty expertise in incontinence, pelvic pain, pelvic prolapse and osteoporosis. Services include an urogynecology clinic just for women, women’s physical therapy, and minimally invasive options including robotic surgery as alternatives for traditional hysterectomy.

Geriatrics: Legacy Health offers expertise in caring for senior citizens, from primary care to the hospital. Several of our clinics have geriatricians on staff. Special services are a hip fracture service, balance assessment and fall risk reduction and a special protocol used to identify and treat elderly hospital patients at risk for complications. Legacy Good Samaritan has been designated as an "Elder Friendly Business" by Elders In Action, a community resource organization.

Hospice: Legacy Hospice provides physical, emotional, social and spiritual comfort for adults and children who are terminally ill, and for their families. All Legacy Hospice services are accredited by The Joint Commission, the nation's predominant standards-setting and accrediting body for healthcare. Our program is primarily home-based, but care is also provided in residential or assisted living settings, adult foster homes and nursing care facilities.
